All Age Continuing Care can be provided in a variety of settings outside of hospital, such as in your own home or in a care home. It covers all identified healthcare costs and is not means-tested. This means it is not based on what you earn, or your savings.
NHS Continuing Healthcare is based on a person’s care needs and not on a particular health diagnosis, disease or condition.
If you feel that you, or someone you care for may be eligible for NHS Continuing Healthcare or need an assessment, please contact our All Age Continuing Care Team. They will start by going through an initial checklist assessment to see if you may be eligible.
The ICB will provide your Continuing Healthcare assessment. We will fairly consider all applications and guide you through the process as needed. Assessments are free.
If you have a social worker, or other NHS health professional working with you, they can complete the checklist and submit it to the Integrated Care Board on your behalf.
There are different services for children and young people (up to their eighteenth birthday) and adults.
